NOMADIC NEEDLEWORK — TATTOO LIABILITY WAIVER & INFORMED CONSENT
By submitting this booking form, I ("Client") acknowledge and agree to the following:
1. AGE & ELIGIBILITY
I confirm I am 18 years of age or older and will provide valid government-issued photo ID on arrival. Tattooing is not performed on minors under any circumstances.
2. VOLUNTARY CONSENT
I voluntarily consent to receive a tattoo from Nomadic Needlework / Know1. I understand tattooing is a permanent body modification and results are irreversible without professional laser removal.
3. HEALTH DISCLOSURE
I confirm I am in good general health and do not have conditions that may complicate tattooing or healing, including but not limited to: blood clotting disorders, active skin conditions at the tattoo site, unmanaged diabetes, immunodeficiency, pregnancy, or active infections.
4. RISK ACKNOWLEDGMENT
I understand and accept inherent risks of tattooing including: infection, allergic reactions, scarring, fading over time, and dissatisfaction with placement or design. Nomadic Needlework is not liable for complications arising from improper aftercare.
5. AFTERCARE RESPONSIBILITY
I agree to follow all aftercare instructions provided by Know1. Failure to do so may result in infection, color loss, or other complications for which Nomadic Needlework bears no responsibility.
6. DESIGN & PLACEMENT
I am responsible for approving the final design and placement before tattooing begins. Once tattooing has commenced, all payments made are non-refundable.
7. DEPOSIT POLICY
The $100 deposit is non-refundable and non-transferable. Cancellations less than 48 hours before the appointment forfeit the deposit. Rescheduling is permitted with at least 48 hours notice, subject to artist availability.
8. PHOTO RELEASE
I grant Nomadic Needlework permission to photograph the completed tattoo for promotional use. Notify Know1 prior to your session if you do not wish to be photographed.
9. RELEASE OF LIABILITY
I hereby release, waive, and discharge Nomadic Needlework, its artist(s), and affiliates from any and all claims arising out of the tattooing services provided, to the fullest extent permitted by law.
10. GOVERNING LAW
This agreement is governed by the laws of the State of Louisiana. Disputes shall be resolved in the courts of St. Landry Parish, Louisiana.
